Overview

This page covers iRest (Integrative Restoration), the protocol derived from yoga nidra by Richard Miller and taught by the iRest Institute. It sets out the ten stages as published, how iRest relates to traditional yoga nidra, its use in clinical, military and veteran settings, what studies have and have not shown, trauma-related cautions, and the institute's training and certification. It is a member of the Yoga Styles family on Machipedia.

Where does iRest Yoga Nidra come from?

Yoga nidra before iRest

A narrative review in Sleep and Vigilance describes yoga nidra as a relaxation technique with components known since early texts, and cites passages from the Upanishads, the Vijñāna Bhairava (around the seventh century CE) and later Hatha Yoga texts that describe lying down in corpse pose. It states that a systematized form reached the public in the 1960s through the writings of Swami Satyananda Saraswati. The review is narrative rather than systematic, and its dating of early sources is secondary, so these dates are best read as one scholarly account.

The same review adds that many trainers have altered yoga nidra and that the classic form has been lost in many settings. Yoga nidra therefore names a family of practices, not a single fixed procedure. The Machipedia page Nidra Yoga covers that wider tradition.

Richard Miller and the iRest Institute

The iRest Institute describes iRest as a contemporary adaptation of yoga nidra that shares many basic principles and techniques with it. It credits Richard Miller, whom it describes as a clinical psychologist, author and yogic scholar, with combining traditional yogic practice with Western psychology and neuroscience. An institute podcast page names Miller as its founding president and CEO and says he has spent over 40 years integrating nondual teachings with Western psychology.

The pages reviewed do not give a date for when the protocol was first formalized, and they do not say whether the organization was previously known by another name. Those points are left open here.

Adoption in military and veteran settings

The institute states that iRest has historically been endorsed by the U.S. Army Surgeon General and that the Defense Centers of Excellence for Psychological Health and Traumatic Brain Injury recognize it as a complementary practice warranting further research on post-traumatic stress. These are the institute's own statements, and the agency documents were not reviewed for this page.

Published reports confirm use in veteran settings. A 2011 feasibility study offered weekly iRest classes to combat veterans at a community mental health agency in the San Francisco Bay Area, a 2014 pilot ran a class for women with sexual trauma at a VA medical center, and a 2015 report describes a VA Integrative Health and Wellness clinic that offered iRest yoga nidra among several services.

What do the names and terms mean?

Names and spellings

The institute writes the name as iRest, short for Integrative Restoration, and publications use iRest, iRest Yoga Nidra, iRest meditation and Integrative Restoration. The institute describes the protocol as a 10-step protocol or framework.

Relationship to yoga nidra and Nidra Yoga

The Satyananda-style structure summarized in the narrative review moves through a resolution (sankalpa), rotation of consciousness through the body, breath awareness, paired opposite sensations, visualization, a repeated resolution and a gradual return to waking. The iRest protocol, as the institute lists it, also begins with a heartfelt desire and an intention and then moves through body, breath, emotions, thoughts, joy and rest in awareness. No source reviewed compares the two directly, so any statement of difference rests on these two separate descriptions.

The institute presents iRest as adapted for modern life and teaches it for personal practice, professional teaching and clinical practice. Traditional yoga nidra as taught in yoga lineages is covered by the Machipedia page Nidra Yoga and, as part of the wider family of yoga approaches, by Yoga Styles. This page covers the iRest protocol only.

Related practices on Machipedia

iRest is a form of guided meditation, and readers may also want the Machipedia pages Meditation, Guided Meditation and Guided Imagery. Pages on Yoga, Restorative Yoga and Trauma-Informed Yoga describe neighboring approaches. The institute presents iRest as derived from yoga nidra, which places it within the wider field of yoga approaches.

What does iRest Yoga Nidra involve?

The ten stages as published

The institute's protocol page lists ten stages. They are Connect to Your Heartfelt Desire, Set an Intention, Find Your Inner Resource, Feel Your Body, and Become Aware of Your Breath. The page describes the heartfelt desire as a deeply held wish felt as if already true, the intention as a purpose for the session, the inner resource as a bodily sense of safety and calm that can be returned to, body sensing as moving attention slowly through the body, and breath awareness as observing the breath.

The remaining stages are Welcome Your Emotions, Witness Your Thoughts, Experience Joy, Find Lasting Peace, and Reflect on Your Practice. The page describes noticing feelings and thoughts without judging them, inviting well-being, setting thinking aside to rest in aware presence, and reviewing the session before returning to daily life. These are the institute's descriptions of its own practice and are not tested claims.

Formats used in studies

Published studies used varied formats. A VA class for women met for 90 minutes twice a week for 10 weeks, a community agency offered weekly classes for eight weeks, and a college class ran for eight weeks. A randomized pilot used a two-day retreat followed by 20 minutes of daily home practice for six weeks, and an Australian hospital pain service ran a six-week group program.

Training and certification

The institute states that Level 2 is a five-day live online training that follows Level 1 and that live participation each day is required for its certificate. Level 2 includes trauma-informed practice and teaching for special groups such as trauma and pain. The institute adds that licensed professionals should check with their own boards about continuing education credit.

Certification is a separate 250-hour program that requires Level 1 (30 hours), Level 2 (30 hours), a short immersion or retreat, a long retreat, about 200 hours of self-study assignments including recorded sessions, and four to five supervision sessions. Certified teachers must follow the institute's Code of Ethics and Scope of Practice. This is the institute's own credential, and the pages reviewed name no external accrediting body or statutory standard.

What is iRest Yoga Nidra used for?

Uses stated by the institute

The institute describes iRest as simple to learn and open to anyone regardless of physical ability or meditation experience, and states that trials have shown benefits for chronic pain, sleep problems, depression, anxiety and post-traumatic stress. The next section reviews what the published studies in fact examined. The institute's statement is a claim by the organization that trains and certifies teachers.

Settings and groups in published reports

Reports describe iRest offered to combat veterans with PTSD, women with sexual trauma in a VA service, veterans in a VA integrative clinic, older adults with depression symptoms, college students, health care workers and people with persistent pain in a hospital outpatient service. Some participants in the pain program said they felt motivated to use iRest in place of medication, which is a participant report and not a clinical recommendation.

What does the research show?

Uncontrolled and early studies

Most iRest studies are small and lack a comparison group. The 2011 feasibility study enrolled 16 male combat veterans and 11 completed it, reporting reduced rage, anxiety and emotional reactivity alongside difficulties with focus and intrusive memories. In the VA pilot for women with sexual trauma, 16 were recruited, 15 enrolled and 10 completed, and completers reported lower PTSD symptom scores, self-blame and depression scores.

A pre-post study of 66 college students reported lower stress, worry and depression scores, and a survey of 15 health care workers found higher mindfulness scores but no significant change in sleepiness or pain. Without control groups these designs cannot show that iRest caused the changes, and completers may differ from those who dropped out.

A randomized pilot in older adults

One randomized pilot assigned 30 older adults with depression symptoms to a two-day iRest retreat or a vacation retreat, followed by six weeks of home practice with guided meditations or music. The authors found the program feasible and acceptable, with improved sleep impairment compared with the control and promising trends for depression and pain. At 6 and 12 months, 25 participants responded. Both groups improved on depression scores, and negative mood and perceived stress improved more in the vacation group.

Veterans, pain and service use

The searches run for this page were PubMed queries for iRest and yoga nidra, and for Integrative Restoration with veterans, PTSD, sleep and pain. They returned 11 iRest-related records and no randomized trial of iRest for PTSD in veterans among the sources reviewed. The VA clinic report describes service use in a first year of 740 consults and 325 enrollees, with acupuncture used more than iRest, so it does not measure iRest outcomes. A 2025 qualitative study reported that participants in a pain program described personal benefits, including self-management and reduced identification with pain, without measuring pain scores.

Reviews of yoga nidra for context

An integrative review of 29 yoga nidra studies, 12 of them randomized, found mostly favorable results but wide variation in samples and sessions. A meta-analysis of 73 studies on stress, anxiety and depression and one of 12 studies on pain described low methodological quality, and the pain analysis found a difference from passive comparators but not from active ones. A sleep meta-analysis rated certainty as very low across five unique studies. These reviews combine many protocols, so they cannot be taken as evidence about iRest in particular.

What are the safety considerations?

Distress when attending to the body and emotions

The protocol includes stages that direct attention to body sensations, emotions, thoughts and memories. In the combat veteran feasibility study, participants reported challenges including intrusive memories, and in the VA pilot five of 15 enrollees dropped out for transportation reasons. None of the iRest studies reviewed here systematically measured adverse events, so the frequency of distress during iRest is unknown from these sources.

A systematic review of 83 meditation studies, excluding yoga postures, found at least one adverse event reported in 65 percent, with an overall prevalence of 8.3 percent, most often anxiety, depression and cognitive anomalies, including in people with no mental health history. A mixed-methods study of 60 Western Buddhist practitioners selected because they had difficulties documented a wide range of effects. Neither study isolated iRest, and the second cannot show how common difficulties are. A review of six yoga nidra sleep trials noted that adverse event and safety data were reported inaccurately.

Care boundaries

The narrative review notes that advocates describe yoga nidra as suitable for everyone, which it presents as a claim and not a tested finding. People with PTSD, depression, persistent pain or sleep problems should not delay or replace necessary care with iRest, and changes to medication belong with the prescriber. The studies reviewed here took place in supervised programs, such as a VA clinic or a hospital pain service.

The institute states that Level 2 covers trauma-informed practice, but the page does not define clinical scope of practice or who may use iRest with clinical clients. The sources reviewed do not describe a statutory standard for iRest teaching. Claims of endorsement and trial benefit come from the institute, and the controlled evidence reviewed is limited to one small randomized pilot.
